You can download the client intake forms from this web page and bring the completed forms with you, in order to allow  more time in your intake.  Your clinician will verify your insurance coverage prior to your appointment.

In order to verify your insurance coverage, the following information is needed

​

• Client's name and date of birth

• The primary subscriber's name as it appears on the insurance card, date of birth, and relationship to the client

• Insurance Company

• Subscriber ID#

• Provider phone number for the insurance company, which is usually found on the back of the card

• Phone number where you can be reached if any questions arise

Initial visit FAQ's

​

Do you work nights and weekends?
Our clinicians are available days, nights, and weekends.  Check with the individual clinicians for their current availability.

 

How long will the appointment last?
The initial evaluation will last for 60 minutes.

Do I need to bring anything with me?

Bring your insurance card and the intake forms to your appointment.

​

Is there available parking?

There is a free parking lot on the east side of the building.

​

Do you offer reduced rates?

Sliding scale rates are available on a case by case basis.

FEES
$200 - 60 minute initial intake

$175 - 50 minute session

TBD -  Psychological Testing (This is determined on a case by case basis depending on the tests administered).

Medicare fees are based on current Medicare allowable rates.

Insurance
We are out-of-network providers for most  insurance companies.   We are in-network for those who have Medicare.  As a service to you, we will submit your bill for your reimbursement on a weekly basis.

 

Payment
Cash, check or credit card at the time of the session.

 

 

Cancellation Policy
Your appointment time is reserved just for you.  There is a 48 hour cancellation policy.  Otherwise, you will be charged for your session.  Insurance plans will not reimburse a missed appointment.  Illness and emergencies are dealt with on an individual basis with your therapist.
